Home Health Aides (Cleveland)

Sweet Home Healthcare of Ohio, LLC is looking for qualified home health aides to provide services to people in need in their own homes. The ideal candidate will:

• Provide for the personal care needs of the patient as designated on the home health aid plan of care. Personal Care duties including but not limited to; personal hygiene and care, patient mobility, elimination and nutrition.
• Report changes in the patient’s condition; medical, living or needs in a timely fashion. These change are not limited to services you provide, if you witness or made aware of anything such as abuse , misappropriation of funds etc you must notify the scheduling departent immediately and come into the office within 24 hours
• Document patient care via Axis Care mobile application
• Perform all duties within the boundaries of the personal care policies and procedures.
• Provide care to the patient with attention to safety needs to the patient.
• Perform household duties as designated by the plan of care.
• Maintain a safe and clean patient environment.
• Encourage appropriate self-care and assisted independence.
• Utilize equipment and supplies appropriately.
• Possess the ability to understand the written activity plans, execute instructions and document services delivered.
• Possess the ability to communicate and access emergency service systems.
• Possess the ability to access transportation services required for task completion.
• Accompany (NOT TRANSPORT) client to appointments as permitted by plan of care.
• Demonstrates a high degree of commitment to customer service and the provision of quality care.
• Ability to lift up to 50 pounds unassisted and up to 325 pounds utilizing as assisting device.
• Exhibit positive interpersonal verbal and written skills.
• Perform 12 Hours of In-services each year
• PASSPORT/ PC aides must attend a minimum of 8 hours of in-services each year and 60 hours of training prior to hire.
• Participate in quality improvement activities and staff meeting, as needed.
• Perform other duties as assigned.

Job Types: Full-time, Part-time

Salary: $12.00 – $14.00 per hour
