Monday-Friday 9 am-5 pm, per diem weekend rotation, holiday rotation.

The LPN Triage Nurse is responsible for gathering and reviewing clinical information to facilitate the overall coordination of community admission visits. The Triage Nurse works collaboratively with the intake team and managers to triage and prioritize visits based on the needs of the referred patient and family. The Triage Nurse communicates with patients, families, and referring physicians to collate the information necessary for the determination of hospice medical eligibility. The LPN triage nurse may also be called on to perform informational visits in the community setting.

Essential Duties and Responsibilities:

  • Provides administrative and clinical support to the admission team nurses in the field.
  • Assists the field staff with phones calls to pharmacies, physician offices and other community healthcare settings to ensure the field RNs have access to all necessary clinical records and information to support the home visits.
  • Triage referrals in collaboration with the RN team Manager and Director for medical appropriateness for the hospice and/or palliative care programs.
  • Enters ICD-10 codes for each scheduled admission visit.
  • Assist admission RN with clinically complex visits as needed.
  • Occasionally called on to perform in home informational visits as needed.
  • Assist the intake team with the referral process when time allows, including incoming phone calls, obtaining insurance authorizations, and uploading medical records into the EMR.
  • Compiles and disseminates the daily admission report.
  • Adheres to requirements of organizational policies and procedures that are both patient focused (clinical) and organizational focused (administrative).
  • Adheres to HIPPA regulations and policies. Maintains security of the patient electronic/written medical records.

Qualifications:

  • Education/Certification
  • Graduate of an accredited School of Practical Nursing with current LPN licensure in the State of NY
  • Experience
  • 1 year of nursing practice as a Licensed Practical Nurse preferr
